Sample Accident Report

In Ontario, any accident that results in a critical injury must be reported to the Ministry of Labour, Training, and Skills Development within 48 hours.

This sample template will help employers understand what to include in the accident report to comply with the requirements under Section 51 of the Occupational Health and Safety Act (OHSA).
